Based on the announcement from the University Student Association regarding the review and statistics of students approaching the title “Five-Good Student,” the Faculty Youth Union of the Faculty of Food Science and Technology (LCĐ) informs all students as follows:
I. PURPOSE AND REQUIREMENTS

Purpose
- To survey and accurately identify the number of students approaching the title “Five-Good Student” for the academic year 2024 – 2025.
- To serve as the basis for evaluating the title “Five-Good Student” and supporting students participating in the English mock test in October 2025.

Requirements
- Class Executive Committees must strictly implement and clearly disseminate the information to all students.
- Students are responsible for completing the survey within the specified deadline.
- Class Executive Committees must manage, monitor, and fully report the results, ensuring 100% student participation.
II. TARGET GROUP AND IMPLEMENTATION METHOD

Target group
- Mandatory for all students of cohorts 13DH, 14DH, 15DH, and 16DH under the Faculty of Food Science and Technology.

Implementation method
- Each student must complete the online survey at the following link: https://forms.gle/YwJN76ukg7CB7cf3A or scan the QR code.
- Class Executive Committees are responsible for disseminating the information to all students in their class, monitoring progress, urging completion, and reporting results to the LCĐ Executive Committee in charge.
- Deadline for completion: Before 16:00, October 01, 2025.
Note: For cohort 16DH students, enter the high school graduation exam score (GPA) as the admission score, and enter the target conduct point for Semester I of the academic year 2025 – 2026.
IV. ORGANIZATION OF IMPLEMENTATION
- Survey information will serve as an important basis for evaluating the title “Five-Good Student” and supporting students participating in the English mock test in October 2025.
- Classes are required to implement the announcement seriously and coordinate fully to ensure quality and student benefits.
- Students who do not comply with this announcement will be deducted conduct points according to regulations (5 conduct points per violation).
This is the announcement from the Executive Committee of the Faculty Youth Union of the Faculty of Food Science and Technology. Class Youth Unions are required to implement it thoroughly, communicate it to all members and students, and ensure compliance with all contents of this notice.
